Many retirement homes are not set up to provide the level of care a resident may need as their health changes. Eldercare Home Health provides one-on-one personalised care inside retirement homes across Toronto, Mississauga, and the GTA.
When the residence needs more
Many of our clients moved to a retirement residence at a time when they were relatively independent. We are usually called when there has been a change in health status that the facility is not equipped to address alone.
We have positive working relationships with the retirement residences where our clients live. We work alongside their staff to complement and supplement the care they provide.
One-on-one, independent care
We answer directly to you. This independence allows us, when necessary, to advocate on behalf of our clients within the retirement home setting.
We are often successful in helping clients continue to take meals in the dining room by providing the support they need to get there and participate. We also assist with a full range of activities of daily living, personal care, and companionship.
Helping residents stay engaged and safe
Our Registered Nurse supervised PSW care makes it possible for your parent to continue to enjoy the many social and physical benefits of living in a retirement residence. We can help your parent with activities of daily living, personal care, meals, and more, including in the assisted living areas.
Many of our clients happily chose to move to a retirement residence at a time when they were able to participate in the various social programmes and activities on offer. We are usually called on to become involved when there has been a change in the health status of the resident, a visit to the hospital, or a cognitive decline. We have very positive relationships with the retirement residences where our clients live. We are, however, independent of the home or residence.
Our goal is to assist our clients to be as safe and independent as possible. We provide care in retirement homes in Toronto, the GTA, and Mississauga. We are often successful in helping our clients find a way to continue to take meals in the main dining rooms with their friends, and to participate in the various activities on offer by providing the support they need to get there. We also assist seniors who are residents in retirement homes with a range of activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
Care inside a retirement residence starts with a Registered Nurse assessment, where we meet your parent, review their health status, and speak with the residence's wellness team so our support complements the care already in place. From there a nursing-supervised plan sets out the hours and the caregiver match. The steps below set out how care begins.
Five steps to care
Here is what happens when you contact Eldercare Home Health about care in a retirement home.

Initial enquiry
We ask about the person you are calling about, any recent changes in health, your key concerns, and how soon care is needed. A Registered Nurse is usually available to speak with you directly.

Assessment by a Registered Nurse
A Registered Nurse visits to meet the client, review their health status and environment, and speak with the care team if in hospital or a facility. All assessments are conducted by a Registered Nurse.

Care plan
The nurse proposes a schedule with identified outcomes, hours, and caregiver type. A Registered Nurse Case Manager is assigned to oversee all care and liaise with the client's physicians and pharmacists. There is no added fee for nursing case management.

Care agreement
You review and sign a care agreement. There is no lock-in; you can cancel with short notice. Invoices are payable by cheque or through your bank.

Care begins
The Registered Nurse Case Manager selects and introduces the caregiver. Our caregivers are Eldercare employees, not contractors. All staff carry liability insurance and are supervised throughout.
