We are there when you need us
Living with congestive heart failure can be very difficult for your elderly parent. Managing the condition requires close attention to medications, diet, fluid balance, and activity levels.
We provide caregivers in the hospital to give you and your family a break. We can provide overnight care and support visits while your parent is admitted. When discharge comes, a Registered Nurse helps coordinate the transition back home and establishes an ongoing care plan.
Registered Nurse case management, included
Managing CHF is complex. Medications must be taken at the right time in the right way. Blood values need monitoring. Diet changes may be required. Exercise must be appropriate.
Our Registered Nurse Case Manager regularly reviews your parent's medications, communicates with their cardiologist and family physician, and monitors for changes that may signal a problem. The goal is to improve quality of life and reduce the chance of hospital readmission.

Eldercare Home Health can provide the relief you need. We'll provide caregivers in the hospital to give you and your family a break. We can provide overnight care, daytime care, or part-time coverage. When it's time to go home — from either an emergency department visit, or a hospital stay — the Registered Nurse Case Manager overseeing care will help ensure that everything you need is in place, so that the transition home goes smoothly.
Our Registered Nurse Case Managers and PSW caregivers can help keep things moving forward once your parent is back home. The right medications need to be taken at the right time in the right way. There are medical and rehab appointments that your parent will need to go to. Your parent's diet may need to change, and an appropriate exercise programme will likely need to be put in place. These are just some of the things that we can help with.
Why non-medical companies cannot help
Over the counter, other prescription medications, and even some foods can get in the way of the medications that may be prescribed for CHF. ‘Non-medical’ companies aren't able to help with these, and other serious issues that can affect your parent's recovery. The Eldercare Home Health Registered Nurse Case Manager will regularly review your parent's medications and consult with your parent's cardiologist and family physician.
The Registered Nurse Case Manager will also communicate with your parent's medical specialists and therapists on a professional level. Our goal is to improve your parent's quality of life and reduce the chance that they will need to return to hospital.
