Our approach

Arranged and overseen by your Registered Nurse

Getting to a clinic for physiotherapy is difficult for many seniors. Eldercare brings the physiotherapist to the home and coordinates treatment with the Registered Nurse case manager, so assessment, exercises, and follow-up are part of a joined-up plan.

No clinic travel required

A physiotherapist visits your parent at home. This removes the burden of transport for seniors who have had surgery, injury, or limited mobility, and allows treatment in a familiar environment.

Coordinated with nursing oversight

Physiotherapy at Eldercare is coordinated with the Registered Nurse case manager. Clinical changes, exercise goals, and any concerns are shared across the care team so treatment stays safe and on track.

Recovery, chronic pain, and stroke rehabilitation

Eldercare physiotherapists work with seniors recovering from surgery or injury, clients managing chronic pain or musculoskeletal conditions, and those in stroke recovery.

Integrated with the care team

Why coordination with nursing matters

Physiotherapy goals are easier to achieve when the rest of the care team knows what they are. At Eldercare, the physiotherapist works alongside the Registered Nurse case manager and personal support workers. This means the PSW who helps your parent move around the home knows what the physiotherapist is working on.

The Registered Nurse also communicates relevant health changes to the physiotherapist before visits, so treatment is adjusted when clinical status shifts. Read more about the Eldercare home care model.

Why families choose us

Integrated in-home physiotherapy

Eldercare has arranged in-home physiotherapy for clients in Toronto for over 30 years. Physiotherapists work within a care model that includes Registered Nurse case management and personal support workers, so treatment is part of a coordinated plan rather than a standalone booking.

Eldercare is a Veterans Affairs Canada provider and works within an ISO 9001 certified quality system. All staff are screened, insured, bonded, and covered by WSIB. When you call us, a nurse will discuss your parent's situation and explain what physiotherapy can be arranged.

Good to know

Frequently asked questions about in-home physiotherapy

Who delivers physiotherapy through Eldercare?

Eldercare coordinates physiotherapy through registered physiotherapists, who are regulated health professionals in Ontario. They visit your parent at home and work within the broader care plan managed by the Registered Nurse case manager. Eldercare has been arranging in-home physiotherapy in Toronto for over 30 years.

What conditions can physiotherapy at home help with?

Physiotherapy at Eldercare supports recovery from surgery, injury, or stroke, as well as management of chronic pain and musculoskeletal conditions. It also includes safety assessments to reduce fall risk at home and guided exercise to improve strength, balance, and mobility.

How is in-home physiotherapy different from a clinic?

The physiotherapist visits your parent at home, which removes the need for transport. Treatment also happens in the environment where your parent lives day to day, so safety recommendations and exercises are practical for the actual space. Coordination with the Registered Nurse case manager keeps treatment consistent with the overall care plan.

Can physiotherapy be combined with other Eldercare services?

Yes. Many clients receive physiotherapy alongside personal support worker care and Registered Nurse oversight. The physiotherapist communicates treatment goals with the care team, so the PSW supporting your parent with daily mobility knows what exercises are being worked on.

In-depth guide

At-home physiotherapy services in Toronto and Mississauga

Eldercare Home Health, through its parent Integracare Inc., has been providing physiotherapy services in clients' homes in Toronto for over 30 years. Physiotherapists are registered health professionals who treat injury, pain, and chronic illness. Our physiotherapists are closely involved in your recovery and ongoing quality of life.

After an accident, injury, or surgery, home is often the best place for recovery. Eldercare Home Health brings physiotherapy to you. Without physiotherapy, injuries can have lasting and permanent implications. Bringing care home means you or your loved one does not have to struggle with transportation or getting to appointments.

Eldercare Home Health can coordinate all aspects of home care recovery. A Personal Support Worker provides support services and personal care, a Registered Nurse provides wound care if needed, and a physiotherapist leads the rehabilitation — all under the oversight of a Registered Nurse Case Manager.

Physiotherapy for seniors, chronic pain, and stroke recovery

In addition to physiotherapy for injuries, our physiotherapists also work with seniors and clients needing treatment for chronic pain, carpal tunnel syndrome, musculoskeletal conditions, and stroke recovery.

Our physiotherapists work in conjunction with Registered Nurses and Personal Support Workers. Services can include guided exercise or stretching to improve posture and comfort, reduce pain, or improve safety at home. A physiotherapist will work closely with the client, their family, and our care team to develop a plan that suits the client's specific needs.

* Initial assessments are conducted by a Registered Nurse. Case management may be provided by a Registered Nurse Case Manager or Registered Practical Nurse Case Manager. Caregivers are PSWs, have similar qualifications, or are internationally educated nurses.
