In an ideal world, all services would be in place and all details would have been sorted out prior to the client’s discharge from hospital and arrival home.

In reality, this seldom happens. It is not unusual, for example for clients to be discharged home from the hospital without their medication or new prescriptions, or without an exercise schedule, wheelchair etc.

We conduct a prescription review – comparing the list of medications at discharge with the list of medications they were taking before being hospitalized – when discharging clients from the hospital.

We also help to coordinate devices – hospital beds, wheelchairs, walkers, commodes etc. for the return home, so that everything is in place (assuming we are brought on early enough to help make these arrangements), on, or shortly after arriving home.

There are many factors that can make a significant difference to the well being of your parent when they return home, such as: maintaining a clean environment, proper nutrition, mental stimulus, socialization, exercise, monitoring changes in health status, communication between health professionals and family members, coordination of other services etc.

A Registered Nurse is on call 24/7

It is important that your parent’s condition is monitored and that a knowledgeable care professional take appropriate action when health changes occur. We always have a Registered Nurse on call 24/7 so our PSW caregivers have a medical professional they can contact at any time of the day or night.
