Our approach

Palliative care held to a nursing standard

Every palliative care plan at Eldercare is written and supervised by a Registered Nurse Case Manager at no additional charge. Our nurses know the specialised needs of palliative clients and coordinate directly with the wider care team.

Registered Nurse case management

A Registered Nurse assesses your parent, writes the palliative care plan, selects the caregivers, and visits regularly. Nursing oversight is included at no additional charge.

Around-the-clock care at home

Palliative clients can receive continuous care at home, day and night. Eldercare provides 24/7 nursing on-call so your family always has a nurse to reach when questions arise.

Care in the comfort of home

Familiar surroundings allow palliative clients to be more relaxed and to take part in daily activities. Family visits are more frequent and more natural than in an institutional setting.

Nurses who coordinate with your parent's whole care team

Eldercare's Registered Nursing Staff are familiar with the specialised needs of palliative clients and the resources available in the community. As medical professionals, they speak the same language as doctors, respiratory technologists, speech-language pathologists, and pharmacists.

In-depth guide

Palliative care at home in Toronto: nurse-supervised support for your parent

Palliative care focuses on providing relief from the symptoms, pain, and stress of a serious illness, with the goal of improving quality of life for the client and the family. It is not limited to end-of-life care and can be provided at any stage of a serious condition.

Eldercare Home Health can provide around-the-clock, 24-hour palliative care for your elderly parent at home. We can also provide short-term relief for family caregivers who need a rest.

For many palliative clients, the familiar surroundings of home mean they are more relaxed, less stressed, and more able to take part in daily activities. For family it means little or no travel time, more frequent visits, and a setting that feels more normal and less clinical.

Palliative care for your parent at Eldercare is supervised and case managed by our Registered Nursing Staff. We have been providing palliative care for clients in Toronto and the GTA since 1995.
