Hospitals are often stretched for resources. Eldercare Home Health caregivers can assist your hospitalized parent with timely visits to the bathroom, showering, dressing, walking and eating. They can provide companionship and mental stimulation. They can also communicate concerns to the hospital staff and our own Registered Nurses, to help ensure action is taken when required.

Discharge

Discharge can be both an exciting and stressful time –  especially if your parent has had an extended stay at a hospital or rehab facility.

There are many details that need to be taken care of – preparation of the home, making sure that the hospital has provided proper discharge notes, arranging for assistive devices, having prescriptions filled, coordinating transportation, scheduling caregivers, and more. Making sure everything is taken care of is key to making the return home a success. Our Registered Nurse Case Managers can help make your parent’s return home goes as smoothly as possible.
