Eldercare Home Health has been a trusted resource for Social Workers in hospitals throughout the GTA since 1995. We understand the hospital environment and the many challenges, and are here to help make your client’s hospital stay and discharge, go as smoothly as possible.

In hospital

We provide overnight and daytime PSW care so your client’s family can take a break. Our caregivers can assist with meals and toileting, keep your client from pulling out IVs and catheters, and more. Having a caregiver at the bedside can also reduce the risk of falls.

At home

For clients who are returning home, our Nurses will work with hospital staff to help coordinate the discharge.

We’ll also set up, supervise, and case manage the PSW care at home.

Having the right care at home – PSW care that is actively supervised and case managed by Registered Nurses and Registered Practical Nurses – means clients can often be discharged sooner, and are less likely to end up back in Emergency.

And while you may think this level of care would cost more, it doesn’t.
