Together, they create a care plan including regular exercise, preparation of meals and assistance with activities of daily living. RN reviews all current medications with client’s physician. Caregivers matched to client and scheduled. Client discharged the very next day.

How Eldercare Home Health helps: RN from Eldercare Home Health visits the home and works on educating the husband on how to manage blood sugar levels. Puts meal plans in place, schedules activities and eating, monitors blood sugar levels closely. Eldercare Home Health schedules and orients caregivers. Caregivers assist with meal preparation, regular physical activity, and monitoring and charting of blood sugar levels.
Outcome: Client’s blood sugar levels stabilize. Client is safe and visits his wife in the hospital daily..

Eldercare Home Health was founded over 9 years ago by Lisa Wiseman. Lisa is a Registered Nurse with a Baccalaureate degree in Nursing from the University of Toronto. Lisa's focus of study was Gerontology, the study of ageing.
Lisa started Eldercare Home Health because she and her colleagues in Gerontology recognized the need for a home health care provider that would take a holistic approach to providing Elder care and specialize in caring for the unique needs of the elderly.
Early in her career Lisa worked as a home care nurse and as a front-line nurse in some of Canada's foremost geriatric hospitals.
She has worked in management at Queen Elizabeth Hospital and The Baycrest Centre for Geriatric Care, two of Canada's leading centres for geriatric care.
